Shisou, more properly Shisou-aneki (死相姉貴), is the Komachi who appeared in Cookie☆OG at 21:01 behind Alice. She is dinstinguished from the standard Komachi design by the always-present shades over her eyes.
In the Voice Drama
Shisou appears in the scene at the Scarlet Devil Mansion library, where Patchouli and Alice sat together. The scene occurs in between the cooking session of Marisa, Sakuya, and Koakuma, where the three characters struggle to make cookies. Patchouli, sensing danger for Alice as the latter would have to eat the cookies, looked at her and said "I feel like I just saw deadly omens (shisou, 死相) [on her face]." Meanwhile, the scene shows Komachi appearing behind Alice's back, ready to reap the magician's soul.
Patchouli's line could alternatively be interpretted as "I feel like I just saw Shisou." Thus, "Shisou" became the name of this version of Komachi in the voice drama.
"I think... I am the shadow of death"
PCLY's original line being translated back into Japanese from another language. This resulting sentence sounds surprisingly cool, and is Shisou's last line before her fight against DIYUSI in OFFSide.
Portrayals in Cookie☆
Shisou is one of the few unvoiced Cookie☆ characters to receive the "aneki" honorific, which is usually reserved for voice actresses.
Because her bottom half is cut off in the original scene, early fanworks would assign her skirt with different lengths. Later, the length is eventually settled to be right below the knee. A lot of of Shisou's netas revolve around her gaping mouth.
The original scene's artwork drew a scythe that is much shorter than normal ones. Derivative works have given a few explanations to this length, including that (1) Shisou had never held a scythe before, or that (2) she was only protecting Hinase from somebody else.
Since the release of OFFSide:DIYUSI, Shisou received renewed attentions due to her playing a major role as equivalent of Sugar in the series. As a result, her popularity received an unprecedented boom, and she became heavily associated with the series' main character DIYUSI.
Because Shisou is unvoiced in the original story, she is often portrayed as mute. She would then have to communicate via writing text on a notepad she holds. This practice originates from Shiriri's video "喋れないので文字で意思疎通を図る死相姉貴GB.death" (April 14, 2018).
In April 2018, Shisou became a favourite of Shiriri, who began making many MADs and BB materials for her. This led to her further increased popularity in 2019, eventually leading to Shisou becoming a major Cookie☆ character. Another one of Shisou's main content creators is Kokakoi.
Relationships
Since OFFSide, DIYUSI is the character who most frequently appears with Shisou together. They share a very similar background as obscure and unvoiced/barely-voiced characters from the original voice drama being promoted to unprecedented popularity.
Kokakoi's DiyuShiso series features her and DIYUSI hanging out at the isakeya in an after-work setting.
Being a grim reaper, Shisou appearing behind Hinase in the original scene is sometimes seen as showing that she is Hinase's Stand like in JoJo. This resonates with Hinase's own Crazy Psycho Lesbian neta.
As another result of her association with Hinase, Shisou's scythe is sometimes associated with Hinase's weapon the Performer of Dies Irae. Sometimes, Shisou is shown using the same Margueritte-Bois de Justice as her weapon.
In the story of the visual novel Dies Irae, the Margueritte-Bois de Justice is the soul of the heroine Marie Breuil residing in the guillotine that killed her. Marie appears as an existence similar to Stand at the side of the protagonists Ren Fuji, comparable to Shisou's portrayal to Hinase.
Shinokage
シノカゲ ("Shadow of Death")
A Sakenomi-like variant of Shisou. It first appeared on the Seiga as "貸本屋と化した死相姉貴" by マスター (April 13, 2018).
This design is later used by Kokakoi since around March 2019 when Kokakoi began mass-producing Shisou videos. Until that point, she either spoke only in subtitles or was a mute character.
The Shisou Siblings (死相姉弟) is the pairing of Shisou and Rei—who has a minor Komachi role in Halloween☆—as big sister and little brother. It is first seen in the BB theatre "死相姉弟☆" (April 14, 2018) by 赤耳亀. It is a relatively obscure character pairing due to Rei's Komachi role being not very well-known.
A robot character who frequently interacts with Shisou in the works of Ari in 2019. It speaks via the re-compiled audio segments of its anime's song tracks, which is a common technique of humorously delivering character lines in MADs.

Ting♬
A unique bell sound that rang when Shisou appears in the voice drama. It is actually part of the background music, an arrange of "Voile, the Magic Library" known as "Honobono Voile". See the voice drama at 21:03 or the song at 0:19.
Since around May 2019, this sound has been recognised and made into a sound sample.
Koburi Tshirt
While Shisou is usually portrayed as having large boobs, she often wears a t-shirt with "こぶり(small sized)" written on it. This t-shirt first appeared in kuso deta's "Shisou's day off (死相休憩の日)".
Fanatic about ice cream
Shisou is sometimes seen licking ice cream when she stands in the background as a mob character. 石恵 played around with this neta in "Shisou-aneki Who Really Likes Ice Cream (こちらアイス超大好き死相姉貴GB)"（September 19, 2019).
